Summary
We present cross-sectional findings on outpatient care for people with dementia in Berlin and surrounding areas derived from the DemTab project. Descriptive analyses cover guideline adherence, care coordination, and patient/caregiver needs. The results reveal improvement potential in documentation and networking and motivate digital support to strengthen primary care pathways.
