This presentation reports primary care outcomes from a cluster-randomized trial evaluating adherence to dementia care guidelines. We analyze documentation quality, referral patterns, and patient-reported outcomes under guideline-oriented support versus usual care. Findings suggest that structured guideline implementation improves key process measures and may facilitate earlier interventions along the care pathway.
